Emergencies
If you are experiencing an emergency, for example: having thoughts of inflicting harm to yourself or others, CALL 911 or go to your nearest emergency room.
Emails/Calls
Non-emergent ONLY
Will be responded to on business days within 24-48 hours. Dr. Suval is unable to respond to calls while with clients. Email is for non-personal correspondence and should not be assumed to be secure. You can communicate securely with me, and request medication refills, if necessary, through the Luminello patient portal.
Prescription Requests
Require 48 hour notice and only during business hours. Prescriptions will not be filled after business hours, during weekends, or during holidays. There is a fee for prescription refills if you have cancelled or not shown for your appointment.
Lost or stolen prescriptions are never refilled
Financial Policy
Suval Psychiatric Services does not accept insurance.A bill will be generated for you to seek reimbursement from your insurance company. Suval Psychiatric Services is not affiliated with Medicaid or Medicare. Therefore, Medicaid/Medicare will not reimburse you for our services. Payment is accepted in the form of credit card, cash or check. There will be a service fee of $25.00 for returned checks due to insufficient funds.
Fees
Initial appointment: Fee is paid when the appointment is made to secure your appointment.
Follow up appointment
Fee is paid on the day of the appointment.
No Shows/Cancellations
If you do not come to your appointment or cancel less than 48 hours before your appointment you will be charged half the appointment fee.
Prescriptions
Requests require 48 hour notice.

Forms and Letters
Paperwork necessary for insurance or employer $ 25.00
